The petitions are presented on plain paper or via the digital platform with a legible indication of the surname, name, date and place of birth and residence and identification documents of the presenters. Presenters residing in other Municipalities must attach to the petitions a specific declaration in lieu of an affidavit made pursuant to article 47 of Presidential Decree no. 28 December 2000. 445, regarding the existence of the continuous relationship with the municipal territory, indicating its nature.
In case of collection of a mixed petition (online and paper) the indication of the tax code must be integrated into the data collected. 

The petitions must be signed by at least 1000 (one thousand) citizens.

The relevant responses are published in the Notice Board of the Municipality of Milan and transmitted to the President of the City Council for forwarding to the competent Commission.
